A UN inquiry has found that a lack of leadership in the UN peacekeeping mission in South Sudan culminated in a "chaotic and ineffective response" to the recent outbreak of violence in Juba.

The investigation set up by UN Secretary-General Ban Ki-moon found that peacekeepers maintained a "risk-averse posture" during the heavy fighting in the capital from July 8 to 11.
