Ronaldo stepped up after Atletico's Juanfran was denied by the left post in the shoot-out as Real won the elite event an 11th time on a another night of heartbreak for Atletico.
The score was locked at 1-1 after 90 and 120 minutes. Man of the Match Sergio Ramos poked home 15 minutes into the Milan final, Atletico missed a penalty through Antoine Griezmann early in the second half but substitute Yannick Carrasco levelled in the 79th, dpa reported.
Real coach Zinedine Zidane - in charge since January - became the seventh man to lift the prestigious trophy as a player and coach.
Atletico are the first club to lose three finals in the competition without ever winning the trophy, after losing dramatically against Real two years ago and Bayern Munich in 1974.
